This one slipped me by - Google have introduced as part of Google Docs an online form builder. You can edit and build all sorts of forms with different form elements, and then embed the forms in emails and webpages.
What is really cool is that the form submissions are then collated in a Google spreadsheet for you. To create one, just go to google docs, and use the drop down new document button, and choose ‘Form‘.
You can embed them in a webpage using an iframe, and edit the confirmation screen to suit too as per my example (Wordpress wouldn’t let me embed an iframe, so the formatting is a bit out with no left margin, but the form works).
A really simple idea brilliantly executed!
